Community Health Worker (San Diego)

Employment Type: Full Time              Hours/Per Week: 40

Cayenne Wellness Center and Children’s Foundation (CWC) is a 501(c)3 organization dedicated to improving health outcomes and decreasing mortality related to Sickle Cell Disease (SCD). Our mission is to increase the quality of life for individuals diagnosed with SCD in California by ensuring expert, unbiased, and comprehensive care. POSITION SUMMARY: 

Under the direction of the Executive Director of Cayenne Wellness Center, the Community Health Worker (CHW), will work on behalf of various grants to:
  1. provide health education and health promotion using evidence-based information for SCD and related issues;
  2. develop partnerships with family organizations to ensure that families are empowered as partners in their care;
  3. assist families with obtaining SCD educational and social support services;
  4. assist with transition services; and
  5. engage in activities that help ensure individuals with SCD have access to services, including a medical home, from the time of diagnosis through the lifespan.
The CHW will be responsible for assisting persons diagnosed with SCD to become advocates through empowerment. CHWs will do so by utilizing Cayenne Wellness Center databases, working with hospital and medical centers, state agencies that provide newborn screening follow-up, hospitals, schools, churches, and other CBOs that provide services to patients with SCD.

CORE DUTIES: 

  • Identify persons diagnosed with Sickle Cell Disease (at minimum three persons/month) and coordinate access to a medical home within two months
  • Provide all social support services as needed (i.e., transportation, counseling, insurance, and housing).
  • Educate patients on the latest FDA SCD modifying therapies
  • Build partnerships with various entities in designated regions (i.e, hospitals, schools, churches, state agencies, and other sickle cell CBOs – at least six per year)
  • Assist emerging adults in the transition from pediatric to adult-focused health care (at least twice a year) and assist individuals diagnosed with SCD to be empowered advocates of their health
RESPONSIBILITIES, SKILLS, AND ABILITIES: 
  • Knowledge of basic medical and sickle cell terminology, communities being served and available resources, and confidentiality standards and HIPAA compliance standards
  • Skilled in written, verbal, and interpersonal communication, time management, multi-tasking, managing multiple projects, and excel and shared computer applications
  • Able to work within a team and with peers and with minimal supervision and independently
  • Able to navigate the computer, create spreadsheets, and shared computer
    applications
  • Ability to prioritize and meet deadlines
QUALIFICATIONS:
Minimum Qualifications (REQUIRED):
• Minimum B.A. degree required
• Minimum of 2 years experience in health, social, or community services required

• Proof of CA ID (driver’s license; Proof of auto insurance required) to run tasks

Preferred Qualifications: 

• Bilingual Spanish-English preferred
• Previous experience working in/with local CBOs or agencies.

Staff Accountant

POSITION SUMMARY: 

The Staff Accountant will play a critical role in providing financial record-keeping to support our Executive Director. They will conduct finance-related tasks including supporting processes such as audit preparation, financial record keeping and reconciliation, and other tasks that may be needed. This unique opportunity is ideal for a highly detail-oriented person interested in strengthening their finance skills in an entry-level role.

AGENCY-WIDE DUTIES AND RESPONSIBILITIES:

  • Supports the mission, vision, and values of the Cayenne Wellness Center.
  • Complies willingly with all organizational policies and procedures.
  • Supports all functions that attain and maintain accreditation and compliance with regulatory agencies.

CORE DUTIES:

  • Perform and/or manage all phases of accounting and finance systems, including general ledger, accounts payable, accounts receivable, payroll, cash management, bank accounts reconciliations, and financial reporting and compliance.
  • Prepare monthly, quarterly, and/or yearly financial statements and perform financial analysis for CEO/Medical Director, Directors, and board of directors, as needed.
  • Assist with forecasting, formulation, preparation, analysis and revision of annual budget (s).
  • Provide timely explanations of variances between actual results and forecasts/budgets; provide corrective action recommendations to management as necessary.
  • Prepare monthly reimbursement and financial status reports to agency’s funding sources, or upon request.
  • Assist CEO/Medical Director in personnel and administrative matters as necessary.
  • Work closely with auditors during the agency’s annual independent financial audit.
  • Prepare report to regulatory agencies, and taxing authorities as required.
  • Recommend to CEO/Medical Director actions necessary to maintain a fiscally sound and controlled environment to ensure accurate financial reporting and protection of agency assets.
  • Responsible for planning, directing, and controlling the financial planning process and accounting practices, to ensure overall accuracy and integrity of financial reporting.
  • Prepare and update agency financial policy manual, as needed.
  • Review expenditures/invoices with the Executive Director for approval and prepare checks at least bi-weekly or as needed.
  • Other duties as assigned.

QUALIFICATIONS:

  • Bachelor’s degree plus five years of relevant employment involving grant acquisition, reporting, and compliance.
  • Experience with federal grant procurement requirements.
  • Excellent oral written and presentation skills.
  • Ability to prioritize personal work schedule and priorities using independent judgment.
  • Ability to manage multiple deadlines in a fast-paced environment with multiple priorities.
  • Outstanding organization, project, and time management skills.
  • Demonstrated resourcefulness and flexibility.
  • Willingness to work occasional nights and weekends to meet grant reporting deadlines, if needed
  • Five years of experience

Sickle Cell Social Worker

Core Duties:

  • Attendance at CWCs weekly multidisciplinary team meetings and education program offerings
  • Attend continuing education training necessary to work toward licensure
  • Establish and maintain effective working relations with patients/clients
  •  Evaluate case problems and provide leadership and consultation in the development and implementation of case plans
  • Understand, interpret, and apply regulations and departmental policies and procedures
  • Plan, schedule, and conduct individual and group meetings
  • Provide clinical case management and therapeutic services
  • Interviews patient/patient’s social support system to complete and initiate psychosocial evaluation
  • Provides psychoeducational groups to support patients and the patient’s social support system
  • Provides ongoing assistance to patients/patients’ social support systems in coping with stress related to hospitalization, disability, and chronic/terminal illness
  • Works collaboratively with the patients and their social support system to establish and implement a plan of care with a multidisciplinary team that meets the patient’s identified psychosocial needs
  • Conceptualize, develop, and implement a comprehensive system of care program to support patients and the patient’s social support system (a system of care ranging from medical, psychosocial, dental, educational, legal, housing, food, spiritual, and financial stability)
  • Engage with community stakeholders and agencies to build capacity and collaboration
  • Develop and conceptualize evaluation feedback to determine the effectiveness of services
  • Conduct a 3-5 year strategic plan
  • Conduct outreach to network with local, state, and national organizations that are providing services for patients and patients’ support systems

Responsibilities, Skills, and Abilities:

  •  Functions as a member of the interdisciplinary team and provides Clinical Medical Social Work services to patients and their families to improve or maintain social, emotional, functional and physical health (Continuously)
  • Ability to provide leadership in the development and administration of social work programs
  • Ability to demonstrate the concepts of ethical social work practice
  • Adhere to the NASW Code of Ethics
  • Ability to maintain effective relationships with staff, other disciplines, and community agencies and to interpret programs to the public
  • Ability to participate in recruitment, selection, and orientation of professional social work staff
  • Ability to write meaningful, concise, and accurate reports and correspondence

Qualifications:

  • ASW, LMSW, or LCSW licensure as required in the state of CA
  • Education: Requires Master’s Degree from an accredited school of Social Work (MSW) or higher
  • Requires a level of knowledge of the Social Work practice at the macro level
  • Requires knowledge of community resources and agencies in relation to health, illness, disability, and social determinants of health at the local, state, and national levels
  • Demonstrates cultural humility and comfort with a diverse patient population. Cultural humility is essential
  • Bicultural/Bilingual competency is a plus
  • Ability to work in an interdisciplinary team
  • Computer literacy with the ability to use standard word processing, spreadsheet programs, e-mail (i.e. MS Word, Excel, Outlook), and web browsers
  • Ability to multitask in a dynamic work environment
  • Demonstrates personable skill sets (reliability, dependability, predictability, and an appropriate sense of humor)
  • Excellent organizational, administrative, and training skills
  • Demonstrates related work experience with BIPOC communities
  • Strong sense of discretion and professionalism

  • Familiar with dynamics of chronic illness on patient and family across the life span
  • Experience working with patients and families from disenfranchised communities
  • Familiar with inequities in the healthcare system and its impact on the delivery of services to patients
  • Strong advocacy skills

Certificates, Licenses, Registrations Required:

  • Requires an active ASW or California license as a Licensed Certified Social Worker (LCSW)
  • Insurance panels – you will need to be on insurance panels so we can get reimbursed
