SCD Lives Matter – Newborn Screening Follow-up Program
We are pleased to announce that Cayenne Wellness Center is a sub awardee of the HRSA Newborn Screening Follow-Up Program | Grant No.: SCDAA-NBS2017. This grant will allow us to reach persons diagnosed with sickle cell disease throughout California in an effort to assess their needs, provide community-based services, care coordination, and health education on sickle cell 101, latest treatments and curatives, and best practices for advocating for oneself.
In addition, through our certified Community Health Workers and Independent Contractors, we will build partnerships with hospital and health systems, state government agencies, clinics and other institutions to locate patients throughout the state of California. If you are a patient or know someone that has sickle cell disease, feel free to refer or contact one of our Community Health Workers (CHW) in your region.
